What a superbill is
A superbill is a detailed receipt for therapy. When you're out of network with a client's plan, the client pays you, then sends the superbill to their insurance with the plan's claim form. If they have out-of-network benefits, the plan pays them back its share. Plans need the same facts a regular claim carries, so a plain receipt usually isn't enough: they want your NPI, your tax ID, the diagnosis and service codes, where each session took place, and what was charged and paid.
Many providers give clients one superbill a month, covering every session that month. This tool makes one in a couple of minutes: type your practice details once (tick "remember" and they're there next time), add the client and the sessions, and download the PDF.
What goes on it
- You and your practice: your name and credential, your individual (Type 1) NPI, your group (Type 2) NPI if you bill as a group, your tax ID or EIN, your license number and state, and your office address, phone and email.
- The client: their name, date of birth and address. The plan name and member ID are optional, but they help the plan match the claim.
- Diagnosis codes: up to four ICD-10 codes, the main one first.
- Each service: the date, the CPT code, the place of service, any modifier, the units, your fee and what the client paid.
- Totals of charges, payments and any balance, and a line for your signature and the date.
The common codes, in plain words
| Code | What it's for |
|---|---|
| 90791 | Diagnostic evaluation: the intake assessment |
| 90792 | Diagnostic evaluation that includes medical services, by a prescriber |
| 90832, 90834, 90837 | Individual therapy, by time: 16 to 37 minutes, 38 to 52 minutes, and 53 minutes or more |
| 90846, 90847 | Family or couples therapy, without the client there and with the client there |
| 90853 | Group therapy |
| 90839, 90840 | Crisis therapy: the first hour, then each extra 30 minutes |
| 99213, 99214 | Prescriber visits with an established client (evaluation and management), at lower and moderate complexity |
Place of service says where the session happened: 11 is your office, 10 is telehealth with the client at home, and 02 is telehealth with the client somewhere other than home. Many plans also want modifier 95 on video sessions; check the client's plan. Use the codes your notes support. Code descriptions here are our own short summaries, not the official CPT wording.
Tips that get clients paid
- Suggest your client calls their plan first and asks whether they have out-of-network benefits, what the out-of-network deductible is, and how long they have to file. Our guide to superbills and out-of-network claims walks clients through it.
- Match the dates, codes and fees to your records exactly. A mismatch is the most common reason a plan sends a claim back.
- Sign and date every superbill. Some plans won't process one without a signature.
- If a client is paying for themselves rather than using insurance, they're owed a Good Faith Estimate before their visits: make one with the free Good Faith Estimate maker.
Your client's privacy
Nothing you type leaves your computer. The superbill is built by the page itself, in your browser: there's no account, nothing is uploaded, and nothing about your client is stored. If you tick "remember my practice details", only your practice and provider fields are saved, and only in this browser on this computer. The PDF you save is yours to store and send the way you'd send any client record.
Questions
Who needs a superbill?
Clients who see a provider who's out of network with their insurance, and who want to ask their plan to pay them back. In-network providers bill the plan directly, so their clients don't need one. Clients with HMO or EPO plans often have no out-of-network benefits, so it's worth checking first.
Is this superbill tool really free?
Yes. There's no sign-up, no email and no limit on how many you make. Cima Find Care offers it to Texas providers; if you'd like clients to find and book you online, you can list your practice, but you never have to.
Is what I type private?
Yes. Nothing you type leaves your computer: the page builds the superbill in your browser and sends nothing anywhere. Client details are never saved. Only if you tick "remember my practice details" are your practice and provider fields kept, in this browser only.
How do I save it as a PDF?
Press "Download PDF". Your browser's print window opens; choose "Save as PDF" (or "Microsoft Print to PDF") as the printer and save. Only the superbill prints, on a plain white US Letter page.
What if a self-pay client is billed more than their Good Faith Estimate?
If the bill is $400 or more over the estimate for any provider or facility, the client can start a dispute with the U.S. Department of Health and Human Services within 120 calendar days of the date on the bill. They can learn how at cms.gov/nosurprises or by calling 1-800-985-3059. It's often simplest to talk to the provider first: they can correct the bill to match the estimate.
